When Wound Care Becomes Inaccessible, We Step In

NYC Wound Bridge Inc. ensures access to mobile wound care for patients with serious wounds when coverage barriers leave them without options.

Our Mission

Our mission is to eliminate preventable amputations by closing the systemic gaps that allow pressure injuries (bedsores) and other complex wounds to worsen before patients receive timely care.

About Us

NYC Wound Bridge Inc. is a nonprofit organization dedicated to eliminating preventable amputations by fixing the access failures that medicine alone cannot solve.
Despite the availability of advanced wound care across New York, thousands of patients—including those living with pressure injuries (bedsores), diabetic ulcers, venous ulcers, and other complex wounds—are unable to reach or remain connected to care. Patients served by Medicaid and safety-net systems are especially vulnerable. Missed appointments, delayed referrals, transportation barriers, and fractured transitions between hospital, facility, and home create systemic breakdowns that allow preventable wounds like pressure injuries to escalate into permanent disability.

Who We Are

We are an access-focused nonprofit dedicated to ensuring that patients can reach, receive, and remain connected to advanced wound care. Operating at the intersection of healthcare delivery, logistics, and patient advocacy, we address the systemic gaps that turn treatable wounds into life-altering outcomes. By building the coordination, navigation, and continuity infrastructure that healthcare systems often lack, we help ensure that care works not just in theory—but in real life, for every patient, regardless of mobility, income, or zip code.

How We Create Impact

We identify patients most vulnerable to delayed or disrupted wound care—often those navigating Medicaid, mobility limitations, or complex discharge pathways—and embed targeted access support at critical moments. By aligning providers, payers, transportation, and community resources, we reduce missed care, accelerate treatment timelines, prevent avoidable escalation of pressure injuries and other complex wounds, and stabilize recovery.

Every engagement is designed for measurable outcomes: fewer readmissions, reduced amputations, lower system costs, and improved patient dignity.

Healing Should Never Cost Dignity

Access barriers—not medical limits—are what force patients into life-altering outcomes. We exist to remove those barriers.
